When you schedule your first appointment you will be asked to come early to fill out the forms that will detail your contact information, insurance and medical history so we can have that information on file.

If you like, you can request to have the forms mailed to you so you can fill them out in advance.

If you previously were a patient of another dentist it would be helpful if you had your dental records mailed to us so we have a history of your dental work.  Generally, we’ll suggest that you have your teeth and gums x-rayed at your first appointment – especially if it’s been a while since you’ve been to the dentist. This will help determine if there are any critical issues that require immediate attention.

You’ll receive a personal call from our office a day or two before your appointment as a reminder. If you have any questions at any time, please call our office and we will be happy to help.
